Hearts For The Invisible Charlotte Coalition functions as lead street outreach agency for Mecklenburg County to provide comprehensive HUD defined street outreach services utilizing the Housing First Model. These services we provide are for individuals and families experiencing unsheltered homelessness. The services include but are not limited to connecting individuals and families to shelter, housing, physical and mental health services, substance use treatment, social services(SNAP, MEDICAID, Social Security Benefits, assistance with retrieving vital records, case management, housing navigation, employment assistance, Coordinated Entry and VI SPDAT assessments, entering data into the Homeless Management Information System (HMIS) and conducting Coordinated Entry and VI SPDAT assessments. HFTIC will respond to community referrals within 48-72 business hours, provide outreach services during severe weather events, and Co- lead role the Annual Point In Time Count with Mecklenburg County Community Support Services. HFTIC is committed to joint collaboration with agencies that provide direct services to individuals and families experiencing unsheltered homelessness. Lastly, HFTIC operates two shared living properties with a total of Nine units of affordable housing for men experiencing homelessness or at risk of becoming homeless.
